lang=\"FR\">;<\/span><\/p>\n<p class=\"MsoNormal\" style=\"margin-left:13.05pt;text-indent:.5in\"><span lang=\"FR\">b) curriculum vitae, modelul comun european; <\/span><\/p>\n<p class=\"MsoNormal\" style=\"margin-left:13.05pt;text-indent:.5in\"><span lang=\"FR\">c) copia actului de identitate;<\/span><\/p>\n<p class=\"MsoNormal\" style=\"margin-left:49.05pt\"><span lang=\"FR\">d) medicul de<br \/>\nfamilie al candidatului (<\/span><span lang=\"RO\">adeverin\u0163a care atest\u0103 starea de s\u0103n\u0103tate con\u0163ine, \u00een<br \/>\nclar, num\u0103rul, data, numele emitentului \u015fi calitatea acestuia, \u00een formatul<br \/>\nstandard stabilit de Ministerul S\u0103n\u0103t\u0103\u0163ii Publice<\/span><span lang=\"FR\">);<\/span><\/p>\n<p class=\"MsoNormal\" style=\"margin-left:49.05pt\"><span lang=\"FR\">i) cazierul judiciar<\/span><span style=\"font-family:\" lang=\"FR\"> <\/span><span lang=\"RO\">(acesta poate fi \u00eenlocuit cu o declara\u0163ie pe propria<br \/>\nr\u0103spundere. \u00cen acest caz, candidatul declarat admis la selec\u0163ia dosarelor \u015fi<br \/>\ncare nu a solicitat expres la \u00eenscrierea la concurs preluarea informa\u0163iilor<br \/>\ndirect de la autoritatea sau institu\u0163ia public\u0103 competent\u0103 <u>are obliga\u0163ia de<br \/>\na completa dosarul de concurs cu originalul documentului pe tot parcursul<br \/>\ndesf\u0103\u015fur\u0103rii concursului, dar nu mai t\u00e2rziu de data \u015fi ora organiz\u0103rii<br \/>\ninterviului, sub sanc\u0163iunea neemiterii actului administrativ de numire<\/u>. \u00cen<br \/>\nsitua\u0163ia \u00een care candidatul solicit\u0103 expres la \u00eenscrierea la concurs preluarea<br \/>\ninforma\u0163iilor direct de la autoritatea sau institu\u0163ia public\u0103 competent\u0103,<br \/>\nextrasul de pe cazierul judiciar se solicit\u0103 potrivit legii \u015fi procedurii<br \/>\naprobate la nivel institu\u0163ional);<\/span><span
